Appointments
We know you have many choices when choosing a Dentist in Evanston, IL. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible. If you need to cancel your appointment, we do require at least 24 hours notice to avoid a cancellation fee.
Insurance and Billing
We accept most traditional insurance plans, contact our office to verify acceptance of your plan. We will be happy to file your insurance claims for you. Evanston Dental Care does not participate in Health Management Organizations.
We are in-network with AETNA, AMERITAS, BLUE CROSS BLUE SHIELD, DENTEGRA, DHA, CIGNA (Total plan only), GUARDIAN, HUMANA, METLIFE, PREMIER DENTAL GROUP, PRINCIPAL, UNITED HEALTH CARE (not plans #16, #20) and SUNLIFE.
Payment Options
We accept checks, cash, Zelle, Visa and Mastercard.
